• Proof of Residency (lease agreement or current utility bill)*
• Ear, Eye and Dental Certificate (Form #3300) from Butts County Health Dept or licensed physician
• Immunization Record (Form #3231) from Butts County Health Dept or licensed physician*
• Social Security Card*
• Parent ID*

* Must be presented to complete registration.

Registration is by appointment only. Slots will be filled in the order of the receipt of completed packets.
