Important Notice Regarding Form 1095-C Availability

Due to new legislation passed at the end of 2024, employers classified as Applicable Large Employers (ALEs) are no longer required to distribute Form 1095-C to all employees automatically. Instead, these forms will be provided only upon direct request.

Form 1095-C is not required for individual tax reporting purposes, and employees do not need to submit this form when filing their taxes. However, if you wish to receive a copy of your Form 1095-C, you may request one by contacting Amy Williams or by logging into your personal ESS (Employee Self Service) account to access it.
