Dear Parent/Guardian

The state of Georgia requires a booster dose of MCV4 (Menactra) for any student attending school in their 11" grade school year, once they have turned 16. To attend Jackson High School or any public high school in the state of Georgia your childwill need proof of vaccination prior to beginning their junior year of high schoolBecause this vaccine will be REQUIRED for your child to attend the first day of school, August 3, 2021, we are providing a service to you and your child at school. We have arranged for Southside Medical Center-Butts County office, our partner for telemedicine doctor visits at school, to come to JHS in May to administer the needed vaccines. This service will help to keep your child at school and save you the hassle of taking him/her to receive the vaccine. We have found that in previous years' parents plan on taking care of it during the summer but it can easily be forgotten and their child is not allowed to start school on the first day; we do not want that to happen. We want to help you by getting this taken care of before school is out this school year
